Immunization


IMMUNIZATION REQUIREMENTS

Existing Students:

All existing students should have already met the immunization requirements established by the 2002 Executive Order 803  which required the following vaccines:

  • Measles and Rubella: 1 dose on or after the first birthday OR laboratory test to show proof of immunity (titer).If you have both an immunization and titer, please upload the immunization record only, not both the immunization and the titer.
  • Hepatitis B: students 18 and younger on first day of classes
  • Housing Students: In addition to the requirements listed above, all students moving into Housing/ Residence Halls must also have proof of meningitis vaccine by move-in date. Meningitis Vaccine - Menactra or Menveo meet this requirement. Meningitis B vaccines such as Trumenba or Bexsero will not be accepted as proof.

 

Exemptions:

  • Exemptions will only be made for students with a medical condition which is a contraindication to the Measles, Rubella, or Hepatitis B vaccines.
  • Medical exemptions must be completed by a board certified, licensed physician (MD, DO), Physician Assistant (PA), or Nurse Practitioner (NP) using the approved Medical Exemption Request Form.
  • Only the approved Medical Exemption Request form will be accepted.
  • Medical exemption requests are reviewed by the Chief of Medical Staff and updated manually by Student Health Services staff.
  • No religious or personal belief exemptions will be accepted.

 

Fall 2020 Incoming Students:

All Fall 2020 incoming students must show proof of the following vaccines:

  • Measles, Mumps, and Rubella (MMR): 2 doses on or after the first birthday OR laboratory test to show proof of immunity (titer).If you have both an immunization and titer, please upload the immunization record only, not both the immunization and the titer.
  • Varicella (Chickenpox): 2 doses on or after the first birthday OR laboratory test to show proof of immunity (titer).If you have both an immunization and titer, please upload the immunization record only, not both the immunization and the titer.
  • Tetanus, Diphtheria, and Pertussis (Tdap): One dose on or after the age of 7 years.
  • Hepatitis B: students 18 and younger on first day of classes
  • Meningitis ACY-W (Menactra or Menveo) - Required for students 21 and younger on first day of classes: One dose on or after the age of 16.
  • Meningitis B (Trumemba or Bexsero) - Required for Housing students only: Students who will be living in housing must complete all doses prior to moving into housing.
  • All incoming students must complete a Tuberculosis (TB) risk questionnaire. Incoming students who are at higher risk for TB infection should undergo either skin or blood testing for TB infection within 1 year of entry into CSULB.

To Clear Immunization Requirements:

  1. To begin the process of completing your Immunization Requirement, click on the link to the Patient Portal. You will have access to the Patient Portal beginning March 1, 2020. Please note: you must pay the $150 enrollment deposit in order to be able to upload documents to the Patient Portal. Without the $150 enrollment deposit, you have access to the Patient Portal, but cannot upload the required documents.

ALI Students:

  • To submit proof of all immunization requirements, ALI students should email their records to the immunization nurse at SHS-Vaccine@csulb.edu.
  • If you are unable to get proof of vaccines or are not current on your vaccines, you can receive the immunizations in person at CSULB Student Health Services.
  • It is required that you submit your records in English. If we cannot interpret your records, you may need to take further steps, such as getting laboratory tests to show proof of immunity (titers) or you may need to receive immunizations at the CSULB Student Health Services. Please note that titers will only be accepted for MMR, Varicella, and Hepatitis B (if applicable).

Immunization Clinics at Student Health Services

  • Appointments are recommended for immunizations.
  • Appointments are required for Travel Vaccines.
  • Make an appointment online or call the Student Health Services at 562-985-4771.
  • Check in at Student Health Services.
  • Before receiving an immunization, it is recommended that you eat breakfast or lunch.
  • After receiving an immunization you must wait 20 minutes before leaving the building.

Influenza (seasonal flu vaccine): Free

Tuberculosis Skin Test: Free (CPIE Students: $15 for the visit)

Tuberculosis Blood Test (Quantiferon): $50.00

MMR (Measles, Mumps, Rubella): $77.25 per dose

Hepatitis A (2 shot series): $43.50 per dose

Hepatitis B (3 shot series): $47.00 per dose

Tdap (Tetanus, Diphtheria, Pertussis): $37.25 per dose

Varicella (Chickenpox): $140 per dose

Meningitis ACY-W (Menactra): $118.25 per dose

Meningitis B (Bexsero): $155.25 per dose

Typhoid (shot): $103.00 per dose

Gardasil 9 (3 shot series) $222.75 per dose*
