The above named student is a disabled student receiving services from the Office of Disabled Students Services (DSS). It has been determined that this student is eligible to receive alternative testing accommodations.
DSS has the responsibility of assisting instructors and disabled students with alternative testing accommodations. It is also responsibility of DSS to determine the appropriate amount of time required for each exam (based on the student's disability). Please refer to Dr. Karl Anatol's letter entitled CLASSROOM ACCOMMODATIONS FOR STUDENTS WITH A DISABILITY dated
August 30, 1990. (See Department Chair for a copy).
It is the student's responsibility to discuss their individual needs with you and make the appropriate arrangements. Please provide all of the necessary information on the contract so we can ensure the integrity and security of your exams.
STUDENT MUST ARRANGE EACH EXAM WITH DSS ONE WEEK IN ADVANCE
OF THE DATE THE EXAM IS SCHEDULED.
The student must return the attached form to DSS (BH-270) where it will be kept on file for use during the semester. Please keep the top white copy and this memorandum for your files.
