Faculty – IMPORTANT Changes Beginning Spring 2019

In an effort to preserve the integrity of exams, better serve students with disabilities, and maximize BMAC resources the following changes are in effect beginning Spring 2019. 

  • BMAC will no longer approve alternative test taking locations outside of BMAC with the exception of lab exams, online etc. Contact BMAC-Exams@csulb.edu if you have any questions.
  • BMAC will no longer be picking-up or delivering exams.

CSULB BMAC Student/Professor Test Accommodation Form

  • Faculty will be asked to sign one testing contract per exam where they will fill out the following sections:
    • Section D – Approval for individual Exam Information (alternative test date & time and allowable items (e.g., scratch paper, open book etc.). 
    • Section E – Exam Method of Delivery (4 different options)
      • Professor will e-mail the exam to BMAC-Exams@csulb.edu
      • Student may pick-up the exam in a sealed envelope and deliver it to BMAC.
      • Professor will deliver the exam to BMAC (SSC 110).
      • Professor will fax the exam to BMAC at 562-985-7183 (fax is temporarily unavailable until further notice).
    • Section F – Exam Method of Return (3 different options)
      • Student may return exam in a sealed envelope to the Department Office. (Location: ________________)
      • Professor will pick-up the exam at BMAC (SSC-110) (Pick-up Signature: _________________ Date/Time: _______________).
    • Section G – Professor Information and Confirmation (contact information & signature).
