The first form, Contact Info, updates your contact information, particularly your email address and current work.
With the second form, Alumni Survey 2007, we ask you participate in our self-study, an important process that occurs every 5 years. Both internal and external committees, the latter composed of physicists from outside the university, examine our degree programs and outcomes. As part of this, we need to have your input and feedback.
I (Patrick Kenealy, Chair) guarantee that your input will be anonymous, aggregated with others, and that your name will not be associated with any reports of the data. Any information will come directly to me, and your name will be separated from the information. I will use your name only to check off that you sent the survey, with no other connection recorded.
Both the Info and Survey documents that you download can be filled out quite easily, and then, with a click of the button at the end, be setup to email them directly to the Chair.
A self-study is a key and significant time for a Department, a time to make needed improvements and modernize our offerings. Thank you very much for any help you can give us.
